Why Us
Why Refer to Rise & Reflect?
Care managers and discharge planners choose us because we reduce the burden on their caseloads — not add to it.
Specialized Behavioral Health Focus
Our team is dedicated to therapy and peer support services. When individuals need immediate crisis resources, we help connect them to the appropriate community supports.
Community-Based Support
Services are delivered where individuals live and work — in homes, neighborhoods, and community settings across Franklin, Granville, Vance, and Person Counties.
Telehealth Available
All eligible services are available via telehealth for individuals with transportation or mobility barriers — no travel required.
Person-Centered Approach
Every service plan is built around the individual's goals, strengths, and preferences — not a checklist. Families and caregivers are included.
Responsive Intake
We respond to all referrals within 1–2 business days. Urgent referrals are prioritized. You'll always know where things stand.
Closed-Loop Communication
With written consent, we keep referring providers updated throughout the client's care — care summaries, coordination calls, and PCP participation.
